Supervision for 2 teachers

I am told we can apply for supervision for 2 teachers at the same time that are teaching together. I trust this is correct.
We would like to begin applying for supervision our Sept MBSR class. Does anyoen know if there is a specific application form for 2 teachers, or do we each apply separately and then somehow alert CFM that we are requesting supervision for the 2 of us? Thanks

Thanks Eowyn–we I never heard anything, so I applied as an individual, and my partner is trying to do the same. Also I applied a few weeks ago and have received no reply…Gus
